What You’ll Do
The Family Preservation Care Facilitator will work with the entire family with the goal of preserving the family while providing tools and techniques for building and maintaining relationships.
Family preservation services are designed to work with families who have a substantiated incident of abuse and/or neglect where DCS/probation believes the child(ren) can remain in the home with the support of a family preservation specialist.
The goal is to offer support and resources to the family so the caregivers can provide a safe environment in the home.
Services will be delivered by using at least one evidence-based practice and will provide concrete assistance to support preserving the family and avoiding removal.
Primary duties include:
Weekly safety visits
Home visits at least once a week
Assessing the entire family for any needs
Developing a safety plan
Developing and monitoring a family assessment and treatment plan that supports the family’s goals
Helping the family obtain services needed to support their goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
Advocating for the family and serving as a link between DCS, court, schools, healthcare providers, employers, community agencies and other Oaklawn services
Documenting all family interaction electronically
Additional details:
Caseloads will not exceed 12
Hours will include availability in the evenings and/or weekend
Work directly with probation and Department of Child Services
Provide parent education
Attend Child and Family Team meetings
Complete monthly reports
Qualifications
Minimum Hiring Requirements:
BA/BS in social service or a human services-related field required
Must have a valid driver’s license and proof of insurance
