Reference forms
Provide three references: one from faculty (not in Student Life), one from your current RA/RL (if applicable), and one from another person of your choice. Supply the requested information at the top of the evaluation forms, and sign the release of "right to read the completed evaluation" if you are willing to do so. Then distribute these forms to your references for completion.
