Downloading Fillable Forms

The Transcript Request  form can be filled in electronically and then printed. Adobe Reader 4.0 or higher is required for these forms. To download the Adobe Reader, go to Adobe's site. 

If you have trouble filling the form you may print the request page, complete it by hand and either fax or mail it into the Registrar's Office.

Using Fillable Forms

1. Open the form by selecting the link. If you have Adobe Reader 4.0 or higher, it will open the .pdf document and have it ready for editing.
2. Select the Hand tool if it isn't already selected.
3. Position the cursor (it will change to an I-beam) inside a form field, and click. (NOTE: If the form field is a check box or a radio button, the cursor changes to a pointing hand.)
4. After entering text (or selecting an item, check box, or radio button), do one of the following:
   Press Tab to accept the form field change and go to the next form field.
   Use the mouse to select the next form field.
5. Once you have filled in the appropriate form fields, print out the completed form and mail or fax it in.

• Please note:  if you are using MAC OS X, use Adobe Acrobat Reader, do not use Preview.
