Q. How can I change the amount of tax withheld from my paycheck?

A. Download the IRS' W4 form, fill it out, print it out, sign it, and turn it in to the HR department.

Q. How do I find out my vacation time?

Each full-time administrator and staff employee is eligible for paid vacation time accrued each pay period, calculated according to the schedule for full time employees. Vacation time both available and taken is listed on each pay stub. Employees with questions about their vacation time may contact the Assistant Director of Human Resources at 535-6111 or using email at . Q. What health providers are employees able to select from their health plan?

Employees enrolled in the Goshen College health plan and their covered dependents may select any licensed or otherwise recognized provider for medical services. With membership in the Highmark Blue Cross/Blue Shield Preferred Provider Network, you have access to approximately 97% of national health care providers. You  may go outside the PPO network of providers but will receive less coverage and no price discount. An exception to this rule occurs when an employee or dependent requires emergency service and a network provider is not in the vicinity. Providers and facilities in the Highmark  PPO can be found at the Blue Cross/Blue Shield website.  For more information, please consult the health plan summary description found at www.goshen.edu/policy/ under Benefit Plan Summaries.  Adobe Reader is required. Q. What dental coverage is offered?

Goshen College offers a comprehensive Dental insurance plan that includes: free preventive dental examination and cleaning twice a year, restorations of cavities, orthodontia for children, crowns, root canals, dentures, and other procedures. Coverage for most procedures is 50%, with the orthodontia benefit set at $1,000. The employee pays the entire cost of dental insurance, and family members may be covered at additional cost. Or you may request a copy of the Plan document from the Human Resources office. Most dentists in Elkhart and surrounding counties are covered under this plan. The actual list of dentists participating in the plan may be viewed at www.hri-dho.com.  Employees and family members are eligible upon date of employment.
