Awesome. There are just a few things you should know before jumping in:

There are 2 parts to this form. The first is the application for a Leadership Award (name, address, phone, email). The second is the Personal Experience Portfolio (PEP), which consists of six essay questions that will be used by the selection committee as part of the selection process.

You have choices!

You can complete only the application (due by 5:00pm EST Dec. 15th), and come back later to complete the essay questions (due by 5:00pm EST Jan 4th) or complete both the application and the essay questions in one session.

A word of advice

It is highly recommended that you compose your answers to the Personal Experience Portfolio (referred to more commonly as PEP) questions in a separate program (like Word or Notepad) and then copy/paste your answers here to avoid losing your work.

While you don't have to complete the PEP essay portion of this form now, please continue with this form until you receive a success message.
