General Registration and Membership Forums MUST be submitted by October 27 or a $50 late fee will be assessed. Confirmation and manual will be sent within three(3) weeks of recept of the completed membership form and appropriate fees from:

Note 1: The payment due date is: Monday, NOVEMBER 9th 2009.

The fees for this year are the same as last year. $225 before November 9th.
- Late fees are as follows:
  +$50 Nov 10th to Dec. 31st 2009
  +$100 starting Jan. 1st 2010.
Schools not participating within the past 5 years: $125.

Note 2: Teams advancing to the State Tournament will have an additional Registration Form and a Fee of $100 due after the Regional Tournaments are completed.
